1. Service Description:
Mobile Labs Pro provides mobile phlebotomy and laboratory collection services at the client's location. Services include specimen collection and secure transport to partnering laboratories for analysis.

2. Client Responsibility for Lab Orders:
All lab orders must be provided by a licensed healthcare provider. Clients are responsible for ensuring that their orders are valid and complete.

3. Insurance Coverage Disclaimer:
Mobile Labs Pro does not verify insurance coverage or determine whether specific lab tests are covered by your plan. It is solely the client’s responsibility to confirm insurance eligibility, benefits, and coverage for any lab testing services performed. Mobile Labs Pro is not liable for any denied claims or out-of-pocket expenses incurred.

4. Payment Terms:
Fees for mobile collection services are due at the time of service unless otherwise arranged. Mobile Labs Pro accepts FSA/HSA cards, credit/debit cards, or other approved payment methods.

5. No Medical Advice:
Mobile Labs Pro does not provide diagnostic interpretation or medical advice. Results and health concerns must be discussed directly with your ordering provider.

6. Missed Appointments and Cancellation:
Clients must provide at least 24 hours’ notice to cancel or reschedule. A fee may apply for missed or late-cancelled appointments.

7. Privacy and HIPAA Compliance:
All personal health information (PHI) is handled in accordance with HIPAA regulations. Client information will not be shared without written consent except as required by law.

8. Consent to Service:
By using our services, clients acknowledge and agree to these Terms and Conditions and consent to the collection of specimens by Mobile Labs Pro personnel.

Cancellation
You have up to 2 hours before your appointment to cancel or reschedule. You will receive a 100% refund if you cancel 2 hours or more prior to your scheduled time. If you cancel less than 2 hours before your scheduled time, you will not receive a refund and will incur a service charge of $25 for rescheduling.
Minors (Under the age of 18)
You must be at least 18 years old to obtain lab services from MLP, unless a parent or guardian is present. However MLP may transport your specimen that has already been collected.
Safety
For the safety and well being of all clients and MLP staff, we reserve the right to refuse service when hazards are present. Examples of hazards are unruly pets, violence of any kind (verbal or physical), inclement weather, etc. to be determined by MLP staff.
